Please include the following with your referral:
- Patient demographics
- Insurance information (preferably a copy of the insurance card)
- Diagnostic studies, including any and all MRI, CT, or any other records the patient may have if you are referring to a specific procedure (if available, this is not required).
Our New Patient Scheduling team will respond via fax with appointment info or reasons for inability to schedule.
