When appropriate, your physician may recommend interventional treatments as part of a personalized plan to help manage pain and improve function.

Remote Therapeutic Monitoring 
For eligible patients, Remote Therapeutic Monitoring allows your care team to follow your symptoms, function and progress between visits.

Chronic Care Management 
If you’re living with chronic pain and multiple health conditions, Chronic Care Management provides ongoing care coordination, education, medication support and personalized guidance to help you stay on track with your treatment goals.

Behavioral Health Integration 
Behavioral Health Integration connects eligible patients with additional support for the mental and emotional effects of chronic pain as part of a comprehensive care plan.

Patient Portal 
Our secure patient portal allows you to message your care team, request appointments, access health information and stay connected to your care—all from one convenient place.

24/7 Virtual Agent 
Our virtual assistants are available anytime to answer common questions, help with scheduling needs and connect you with the appropriate member of your care team when needed.

Educational Resources and Whole-Person Support
