Chelsea Cummins, FNP-C

Chelsea earned her Master of Science in Nursing as a Family Nurse Practitioner from Indiana Wesleyan University in Marion, Indiana. She also holds a Bachelor of Science in Nursing from the University of Southern Indiana and an Associate Degree in Nursing from Vincennes University.

Chelsea has extensive experience in family practice and urgent care settings. Prior to her role at Commonwealth, Chelsea practiced at Daviess Community Hospital Quick Care Clinic and Proactive MD at Tommy Wolfe Wellness Center where she provided comprehensive primary care services in a direct primary care model.

Her clinical experience includes diagnosing and treating acute and chronic conditions, preventive and wellness care, physical assessments, and chronic disease management for patients across the lifespan, from newborns to geriatric patients.

Her prior roles also include family practice positions at Memorial Hospital and Health Care Center, CVS MinuteClinic, Evansville Multi-Specialty Clinic, and Daviess Community Hospital. She has also provided procedural care such as trigger point injections, joint injections, cryotherapy, laceration repair, and punch biopsies. Earlier in her career, Chelsea worked as a critical care nurse, inpatient rehabilitation nurse, and staff development specialist and nurse educator.

Chelsea holds active licensure as a Registered Nurse and Advanced Practice Nurse in Indiana, along with prescriptive authority and DEA registration. She is certified as a Family Nurse Practitioner through the American Academy of Nurse Practitioners and maintains Basic Life Support certification.
